Problem definition

Patient Safety is a global concern and should be addressed in a structured way. Many organizations still have a paper-based process for incident reporting and analysis. This process is time consuming and doesn’t give real time insight in the actual situation. Other organizations, that already report incidents digitally, lack the integrated tools for the in-depth analysis of the events or the quality improvement tools. By using the Adverse Event (AHRQ) app you will be able to identify, classify, analyze and improve in the most structured en efficient way.

The Adverse Events (AHRQ compliant) app includes the Healthcare Event Reporting Form (HERF) to capture all information on incidents, near misses, and unsafe conditions. This form is analogous to incident reports commonly used in hospitals today. It contains structured questions and allows for a brief narrative to document the precise details of the event or unsafe condition. Averse Events (AHRQ) will enable your organization to prioritize and analyse the events in a structured and fast way. Corrective and preventive actions can be deployed and monitored and you’ll be able to measure the results of your efforts.)

 


Course of solution

Phase 1 – Identification using the online form (HERF)
Any staff within your organization can report an incident, near miss or unsafe conditions using the online Health Event Reporting Form. Depending on the possible impact, probability and other criteria key personnel will be notified to take (immediate) action. Reporters will also be updated about the status of their notification, which will motivate them to notify even more events.

Phase 2 – Analysis and prioritization
Key personnel will be notified to take action on the risk(s). By logging into the app the flexible dashboard will show all the events including the risk level. The concerning file can be analyzed by the use of analysis tools, like Ishikawa, (automatically generated), Cause tree (5 Why’s), process analysis, barrier analysis and in-depth Root Cause Analysis.

Any task can be monitored and/or assigned to thirds for tracking purpose. Notifications and reminders will be sent by mail so nothing is forgotten and no information will be lost.

Phase 3 – Improve and communicate
After having analyzed the event, corrective and preventive action, action plans etc. can be defined, deployed and monitored by using the standard functionality of TPSC Improve. You can even measure the results of your efforts by configuring the Indicators.
