The 'Adverse Event (WHO)' app is a complete solution for managing your patient safety real-time by using WHO's framework and the International Classification for Patient Safety (ICPS): "the ICPS is designed to be a genuine convergence of international perceptions of the main issues related to patient safety and to facilitate the description, comparison, measurement, monitoring, analysis and interpretation of information to improve patient care."
Adverse Event reporting may not always be subject to a structured, efficient or digitally empowered process. As a result, standardization, formatting, communication, feedback, analysis, learning, response and dissemination of lessons learned may not always be optimized in healthcare organizations. Many organizations still have a paper-based process for incident reporting and analysis. This process is time consuming and doesn’t give real time insight in the actual situation. Other organizations, that all ready report incidents digitally, lack the integrated tools for the in-depth analysis of the events or the quality improvement tools. By using the Adverse Event (WHO) app you will be able to identify, classify, analyze and improve in the most structured en efficient way, while saving costs.
Phase 1 – Detection and reporting of an incident
Any staff within your organization can report an incident, near miss or unsafe conditions using the online form. Depending on the possible impact, probability and other criteria key personnel will be notified to take (immediate) action. Reporters will also be updated about the status of their notification, which will motivate them to notify even more events.
Phase 2 – Analysis
Analysis of reported incidents is a key next step in the process of the Adverse Events (WHO). In order to identify risks and underlying (root) causes, a number of analysis methods are available in the app.
Any task can be monitored and/or assigned to thirds for tracking purpose. Notifications and reminders will be sent by mail so nothing is forgotten and no information will be lost.
Phase 3 – Improve and communicate
After having analyzed the event, corrective and preventive actions, action plans etc. can be defined, deployed and monitored by using the standard functionality of TPSC Improve. You can even measure the results of your efforts by configuring the Indicators.
By implementing the Adverse Event (WHO) app your organization will be able to fully support the fundamental role of patient safety reporting as stated by the 'WHO World Alliance for Patient Safety': enhancing patient safety by learning from failures of the healthcare system.
You will have access to a complete solution for the identification, classification, analysis of events and the improvement of your patient safety. The Adverse Event app will give your organization a complete real-time overview of all your risks and improvement areas.
Your organization will experience a significant increment in the number of notifications, due to the simple and smart online form and the automatic reporter feedback. An increment of at least 300%, compared to paper-based processes, can be expected.
The flexible set-up will give you the possibility to modify the app to your specifc needs. For example by adjusting the centralized workflow to a more decentralized control.
By implementing the Adverse Event app you will even be able to synchronize your data with a national database or Patient Safety Organization (PSO).
The Adverse Event app can be integrated with other apps, like Credentialing, Complaints and Risk Manager, to create a complete framework for Clinical Governance.
Start improving your patient safety today. It will save you money, time and will help you to improve your quality of care and patient safety.
