Prospective analysis

There are different analyses components available to complement your IMS. Basically, they fall into two categories: retrospective (in hindsight) and prospective (predictable).

There is a variety of prospective methods and techniques of which the Health Failure Modes and Effects Analysis(HFMEA) is amongst the most renowned, HFMEA is a systematic and proactive approach to identify ways that a process or design can fail, why it might fail, the effects of that failure, and how it can be made safer. HFMEA focuses on how and when a system will fail, not IF it will fail.

By carrying out a HFMEA, you can assess the relative impact of different failures, in order to identify parts of the process that are in most need of change.

HFMEA includes review of the following:

  • Steps in the process (diagramming/flowcharting the process is recommended)
  • Failure modes (what could go wrong?)
  • Failure causes (why would failure happen?)
  • Failure effects (what would be the consequences of each failure?)
