We, The Patient Safety Company, respect the privacy of everyone who comes into contact with us. Personal information you provide to us will be treated confidentially. Processing of personal data is done in accordance with the requirements of the General Data Protection Regulation. This privacy and cookie statement describes how The Patient Safety Company handles personal data.

For what purposes do we use your personal data?

We obtain various personal data from you. For each purpose we indicate which data we obtain from you, for what purpose we process this data and how long it will be kept. If you have any questions or would like to know exactly what information we keep about you, please contact us. The contact details are at the bottom of this privacy and cookie statement.

Contact and complaint handling

When you send us an e-mail, fill in the contact form on the website, contact us by telephone, or contact us in any other way, you accept our offer to contact us (execution of the agreement).

We process the following personal data for this purpose:

  • Name
  • Business information (if you are contacting us from a business perspective)
  • Contact details (email address and telephone number)
  • Any information that you enter yourself as the content of a message

We store these contact moments with the personal data you provide for the purpose of carrying out the contact and possibly handling your complaint. We keep this information for as long as necessary for this contact or for a maximum of 2 years after we last contacted you, because we want to be sure that we have handled your question or complaint properly.

Newsletters

You can register for The Patient Safety Company's newsletter via our website by giving your consent. We send a bi-monthly e-mail newsletter containing relevant information about patient safety, incident management and quality improvement of The Patient Safety Company.

We process your name and email address for this purpose.

We store your data until you unsubscribe from our newsletter. If you no longer wish to receive the newsletter, you can unsubscribe at any time via the link at the bottom of the newsletter.

Request a demo

If you would like to know more about our services, you can submit a demo request online via our registration form. We need this information to process your request, because you have asked us to contact you about our services. We process this data to carry out the order you have given.

We use the following personal data for this:

  • Name contactperson
  • Company Name
  • Business contact details (email address and telephone number)
  • IP address
  • Any information that you enter yourself as the content of a message

We store this information for up to one year in connection with the quotation process that may take place as a result of the demo.

Providing our services

If you like the demo of our software and decide to purchase our service, we will need additional information from you. We use this data because of the legitimate interest we have in being able to execute the agreement we enter into with your organization and to maintain the relationships that arise from this.

We process the following data for this purpose:

  • Name contact person
  • Company Name
  • Business contact details (email address and telephone number)

In the event of an agreement, we store your data for 2 years after the service ends. We keep financial data for longer if we are legally obliged to do so (for example due to the fiscal retention obligation of 7 years).

References

On our website we present various references about our services, also known as customer cases. In this way we try to give you a complete picture of our products/services and how our customers assess their use in practice. We process data based on the consent of the reviewers. This consent can easily be withdrawn at any time by contacting us via the contact details found at the bottom of this privacy and cookie statement.

We process the following data for this purpose:

  • Name of the reviewer
  • Name of the organization where the reviewer works
  • Possibly personal data as content of the customer case

We need this information to assess the reference and process it on the website. We retain the data until you withdraw your consent to publish the customer case. 

Become a partner

WWe are always working to expand our The Patient Safety Company network with partners who help healthcare institutions use quality and risk management software to improve safety and quality. It is possible to submit an application to become a partner via our website. In order to consider your company as a partner, we do need some personal data. We process this on the basis of the legitimate interest we have, namely the interest in ultimately executing the agreement with the company to become a partner.

We process the following personal data for this purpose:

  • Name contact person
  • Company Name
  • Business contact details (email address and telephone number)
  • IP address
  • Any information that you enter yourself as the content of a message

We store this data for the duration of the collaboration, and up to one year after termination of our collaboration. We keep some data for longer if we are legally obliged to do so (for example due to the tax retention obligation of 7 years).

Working at The Patient Safety Company

Have you responded to one of our vacancies or have you submitted an open application? We then process your personal data in order to process your application and in preparation for possibly concluding an employment contract.

We use the following personal data for this:

  • Name and address details
  • Contact details (email address and telephone number)
  • Curriculum vitae
  • Motivation letter
  • Salary indication, if applicable
  • Any references provided
  • Any other information that you send with your application

We keep your application data for a maximum of 4 weeks after the position has been filled. We store this information so that we can contact you if the position becomes available again during the probationary period. If we cannot offer you a position at this time, we may – with your consent – retain the application data for another year. You can withdraw your consent at any time by sending us an email. If you come to work for us, we will store your application data in the personnel file. This file will be kept for as long as necessary and with regard to the application data for a maximum of 2 years after employment.

Please note: a social media and Google search may be part of the application procedure. This is necessary to ensure that our image is maintained when hiring new staff. We search for your name on Google and your profile on various social media. This of course is insofar as these profiles are public. We will not ask you to grant us access to a protected social media page or to make a connection with us. The results of the research will be discussed with you.

In addition to a social media and Google search, an assessment can also be part of the application procedure. For certain positions it is necessary to guarantee that applicants have certain qualities that are important for the position in question. If an assessment is necessary, we will inform you prior to the assessment about how we will handle your data for this purpose.

Sharing personal data with third parties

The Patient Safety Company does not sell your data to third parties and only provides it if necessary:

  • for the execution of our agreement with you;
  • to comply with a legal obligation (for example if the police require this if a crime is suspected);
  • because we have a legal basis for this;
  • because we have engaged them to process certain data.

The parties that process personal data on our or your behalf are:

  • IT suppliers and service providers
  • Hosting party
  • Cookie service providers
  • Payment service providers and collection agencies
  • Partners
  • External consultants and intermediaries
  • Government services (including the Tax Authorities, municipalities)

Some of our suppliers are located outside the European Economic Area (“EEA”). In that case, we ensure that appropriate safeguards have been taken to protect your privacy.

We enter into a processing agreement with companies that process your data on our behalf to ensure the same level of security and confidentiality of your data.

This privacy and cookie statement applies exclusively to the services of The Patient Safety Company. We are not responsible for the privacy policy of other websites that are accessible via a link on our website.

On our website we use social media buttons, which redirect you to the relevant social media platforms. This gives you the option to follow us and share content within the network. What privacy rights do you have?

You can always contact us if you have any questions about our privacy and cookie statement. In addition, you have the following rights with regard to your personal data:

  • Right of access: you have the right to view the personal data that we process about you.  
  • Right to rectification: you have the right to correct or supplement the personal data that we process about you if it is incorrect or incomplete.    
  • Right to object: you can object to the processing of your personal data.
  • Right to deletion: you can request us to delete your personal data.   
  • Right to withdraw your consent: if you have given us consent to process personal data, you can withdraw this consent at any time.   
  • Right to data portability: if it is technically possible, you have the right to have the personal data that we process about you transferred to a third party.     
  • Right to limit processing: in some cases you can request us to limit the processing of your personal data (temporarily or otherwise).   

We may ask you to identify yourself with your request. We request information for this purpose to ensure that you are the correct person to whom the personal data belongs.

In principle, we will comply with your request within 30 days. However, this period may be extended by two months for reasons related to the specific privacy rights or the complexity of the request.

If we extend this period, we will inform you in a timely manner. If you wish to exercise any of your rights, you can make this known via privacy@patientsafety.com.

Adjustment of privacy and cookie statement

We reserve the right to amend this privacy and cookie statement. Changes will be published on our website. The Patient Safety Company therefore advises you to regularly visit this page to see whether any adjustments have been made.

Submit a question or complaint

If you have any questions or would like to file a complaint about the use of your personal data, you can send an email to privacy@patientsafety.com. We handle every question and complaint internally and communicate this further with you. If you feel that we are not helping you in the right way, you have the right to file a complaint with the supervisory authority.

This is called the Dutch Data Protection Authority.
