Options For Care Ltd is based in Birmingham (UK) and provides rehabilitation to adults over the age of 18 with severe and enduring mental health problems.

Options for Care struggled with the issue of poor incident reporting for some time. Incidents were reported on an ad-hoc basis and they were reported using several different forms or even on scraps of paper.

The problem:

  • Lack of understanding of incidents
  • Lack of structure and policy for reporting
  • Incorrect reporting or incidents not reported
  • Poor standards of information reported
  • Inability to effectively analyse date submitted
  • Leading to an organisational inability to effectively learn from incidents.

As a short-term solution a new policy was drafted which provided clarification on what constituted an incident and when to report. It also enabled staff to use one single form, providing structure as to what information should be provided. An attempt at paper-based analysis was put in place, with monthly report submitted by managers and a plan for quarterly reports to the Quality Governance Board. However, problems remained; still some incidents were not reported indicating a continued lack of understanding of the need for reporting and also what constituted an incident. The standards of information varied between reports making detailed analysis limited impacting on the ability of the organisation to learn from incidents and the process was extremely remained resource intensive.

Our long-term solution was to source an electronic solution to the problem. Through liaison with The Patient Safety Company we were able to build one complete solution for incident management and analysis of near misses, incidents and serious incidents. Every employee can now report a near miss or incident through an easy to fill-in, step-by-step, online form, which takes a few minutes to complete, following the shortcut placed on each computer. The new format supports analysis of incidents and the improvement process that was essential to ensure that risks are reduced and lessons can be learned and shared leading to the quality of the service we required.
